That's why we developed the DocuForms Mix-And-Match Deal. Here's how it works: let's say you're ordering 3 packs of Patient History Reports and 5 packs of Nursing Home forms. That's 8 packages of forms altogether.
Check the per package price of each item in the 8-23 package column. (In this case, the Patient History Reports sell for $35 per package and the Nursing Home forms sell for $50.)
Since you're buying 8 packs altogether, 3 packs of Patient History Reports cost $105 (3x$35) and 5 packs of Nursing Home forms cost $250 (5x$50). The full cost of the order is $355 plus shipping.
Continuing with the above example, if you were to add sixteen additional packages of forms, for a total of 24, you would use the 24-39 package column to price each item. Thus, the Patient Histories would be $30 per package and the Nursing Homes would be $45 per package.
